Insurance Information
Our dietitians are in network with with Blue Cross Blue Shield (BCBS), UnitedHealthcare (UHC), Aetna, Cigna, TRICARE West (TriWest), Centene, Ambetter, Superior Health, and Medicare
If you’re covered by one of the insurance plans listed above, call the number on the back of your card before scheduling your first visit. Ask the representative these key questions to confirm your nutrition benefits:
- Is my plan fully insured or self-insured?
- Does my plan follow the Affordable Care Act (ACA) preventive care guidelines?
- Do I have preventative coverage for Medical Nutrition Therapy (Procedure code 97802 and 97803)
- How many visits are included each year?
- Do I need a referral or prior authorization from my doctor?
- Do I need a specific condition or diagnosis to see a dietitian?
- Do I have a deductible to meet first? If yes, how much?
- What is my copay, coinsurance, or deductible for these visits?
- Are telehealth visits with a registered dietitian covered the same as in-person visits?
- What is my reference number for this call? Write it down and keep it safe—you WILL need it for billing or coverage questions later
If you are not Contracted with the Above Insurances
If you are not contracted with the above insurances and you would like to inquire about out of network reimbursement, call the number on the back of your card and ask the following questions to verify reimbursement before booking your first appointment.
Optional Script
Hello, I am interested in seeing an out-of-network Registered Dietitian Nutritionist.
Does my plan provide reimbursement for out-of-network providers?
If yes, make sure to ask the following questions:
How many sessions does my plan cover, and how long are the sessions?
What is my deductible, and has it been met?
What is the coverage amount per therapy session?
Do I need pre-authorization from my insurance company or PCP?
For your own peace of mind (and organization) make sure to ask your insurance company exactly how to submit your Superbill (the detailed receipt that I will provide you after your sessions) for reimbursement.
After my appointment with my Registered Dietitian Nutritionist, where do I send the Superbill for reimbursement, and when can I expect to be reimbursed?
Pay Policy
If you are planning on receiving out-of-network reimbursement for medical nutrition therapy, or you plan on self-paying for your session, you are responsible for payment at the time of your service. I accept cash, check, or credit/debit card.
